Aftercare and Alumni Support
The plan for what comes next is built weeks before you reach it, and the men you did it with stay in touch. Alumni meet on Tuesday nights, and the group carries on after you leave.

What aftercare means here
What an Aftercare Plan Actually Covers
That is how admissions describes the alternative, and it is the thing families are really asking about when they ask how long the program is.
- 1
A plan built before you leave
A therapist where you are going, groups near you, an outpatient program you can check into a couple of times a week. Case management puts it together with you rather than handing you a leaflet on the last day.
- 2
The brotherhood, still running
Meetings, groups and an alumni app. The men you did this with are the ones who notice when you go quiet, which is the part no aftercare plan on paper can do.
- 3
A door that opens both ways
Men come back. Some to speak, some to sit in, some because a week went badly. Calling is not an admission of failure here and nobody treats it as one.
What an aftercare plan includes
- An aftercare plan built with case management
- Referrals to a therapist where you live
- Local group and meeting connections
- Alumni meetings and groups
- The alumni planner and app
- Reunions and alumni events
- A number that still works after discharge
- A route back into treatment if you need it
What families ask
Life After Rehab, and the Questions Nobody Wants to Say Out Loud
These come up on the phone constantly, usually from a wife or a mother, usually at the end of the call.
What happens on day ninety-one
The honest answer is that it depends on the plan, and the plan is made weeks before the date. Families who ask this early get a better one, so ask early.
If he is going back out of state
Most men here traveled to get here, so most aftercare plans point somewhere else. Case management builds it around the town he is actually returning to.
If he relapses
It is not a disqualification and he will not have to start from the beginning administratively. The first thing to do is call, and the second is to let him call.

- What is aftercare in addiction recovery?
- Aftercare is the plan for what happens once structured treatment ends: a therapist, groups, meetings, sometimes a step down into outpatient treatment. Here it is built with case management before discharge rather than handed over on the last day.
- What does your aftercare plan actually include?
- It is built around where you are going. Typically a therapist near you, local groups and meetings, and often an outpatient program you can attend a couple of times a week. If you are staying in Arizona it may include our virtual program; if you are heading home, it will be services there.
- How long does alumni support last?
- There is no end date. Alumni meetings, groups, the app and the events keep running, and men come back years later. The brotherhood is the part of this program that is meant to outlive the treatment.
- What happens if I relapse?
- Call. It is not a disqualification and it does not put you at the back of a queue. Admissions will work out what level of care makes sense now, which may be residential again, may be PHP, and may be a conversation and a meeting.
- Can I come back to visit?
- Yes, and men do. Some come to speak to the current group, some to sit in, some to help someone else through the door they came through. It is a normal part of how this place works.
- Do you help with sober living after the program?
- During the program, housing is included. After it, admissions and case management will point you at the houses they trust, including Oxford Houses locally and options in your home town.
- I am an alumnus and someone I know needs help. What do I do?
- Call the same number. A meaningful share of the men who come here were sent by someone who had already been. Call the same number you always had.

I was hesitant on coming to this facility, but I'm very glad I did. It definitely enforces the guidelines for building a structured life while having some freedom to still feel like a person. The camaraderie and brothers you make here helped with my recovery exponentially. The staff and therapists at Holdfast take your issues and needs to heart and assist to the best of their abilities.
Alec ReddingVerified Google reviewHoldfast was instrumental in getting my loved one on the road to recovery. Very thankful for the faith based approach while also applying expertise in the field of addiction and recovery. A wonderful ratio of providers to patients for one on one therapy and support. My loved one left after completing a 90 day program with a plan, community and a sponsor. A new lease on life! Our biggest thanks to Holdfast and prayers for the success of your future clients.
Dana KaupieVerified Google reviewHoldFast is a place where God restored me from the inside out. It’s more than a recovery program, it’s a Christ-centered brotherhood that helped me rebuild my life on solid ground. Through prayer, Scripture, and real accountability, I learned how to cling to God in every season and rediscovered my identity in Christ building a life of sobriety. I leave HoldFast renewed, anchored, and grateful for the grace and strength I found here, taking it one day at a time.
Scott ThibodauxVerified Google reviewCombination of Neuro Faith' Therapist, Christian based teachings. Listening and sharing each clients experiences and thoughts about we were being taught in group meetings. Also encouraged to go to AA meetings, Church and Bible Study. I haven't sobriety for 96 Days straight since i was 15. Daily prayer and meditation combined with the Hold Fast App daily reflection schedule allowed me to stay focused on my goals of God and sobriety. I knew no one in Prescott when I moved here, now I have a brotherhood and Fellowship of Men that 100% i can count on. I am currently on Step 11 and plan on Sponsoring others as well as continuing in volunteering for others needs and bringing others to sobriety. I have a beautiful new life now that. Hold Fast brought me back ty rean honest self. Thank you Staff. I am now closer with my family.
