Issued by the State of Arizona. The Arizona Department of Health Services licenses and inspects behavioral health providers, and its Licensing Division will confirm a license for you on (602) 542-1025.

Joint Commission surveyors review a provider against their national standards at least every three years.
What is LegitScript certification and why does it matter?
LegitScript reviews addiction treatment providers for legality, safety and transparency, and monitors them on an ongoing basis. It matters practically as well as ethically: Google requires LegitScript certification before a treatment provider is allowed to advertise at all, which is why a certified provider is a provider that has been looked at by somebody other than itself. Holdfast is certified.
Who is licensed for what?
Holdfast Recovery LLC is licensed by the State of Arizona under license number OTC9025, and delivers partial hospitalization, intensive outpatient and outpatient care. AnchorPoint is a separate organization and delivers the residential level. One admissions team answers for both, which is why the two names come up in the same conversation.
How do I check any of this myself?
Every credential on this page links to the issuer, not to a copy of a logo. The Joint Commission record is searchable by name or by HCO ID, and the LegitScript status can be looked up against our domain. Checking a provider before you hand them someone you love is a reasonable thing to do, and you should do it with more than one of them.
