Almost nobody who calls about kratom set out to get addicted to anything. They started it for a reason. A back injury. Chronic pain. Getting off pain pills without a doctor. Or, as one man put it to admissions, because it gave him peace in his head. It was on the counter at the smoke shop, it was called a supplement, and it worked.

Then it stopped being optional. This page is for the man in that position and for the family trying to work out whether it is really a problem. If you are thinking about coming off it at home this week, read the detox section first.

Is kratom addictive? The kratom addiction signs that matter

Yes, it can be. The FDA says the compounds in kratom act on the same receptors in the brain as opioids like codeine, and it warns about substance use disorder, tolerance and physical dependence. NIDA notes that researchers are still working out how common that is. The men who call here are not the uncommon cases. They are the ones it happened to.

The kratom addiction signs worth taking seriously:

• The amount has crept up, or he has moved to stronger products to get the same effect.
• He feels sick, restless or unable to sleep if he misses a dose.
• He has tried to stop and gone back to it within days.
• It is costing real money, and he is buying it more than once a day.
• He still calls it a supplement.

Families are often split on this one. Somebody in the house says it is herbal and harmless, somebody else has watched a man spend a year and most of his money on it. The FDA has not approved kratom as safe for any medical use, and that settles the argument more usefully than any label on the packet.

What 7-OH is, and what 7-OH withdrawal means for planning

7-OH, short for 7-hydroxymitragynine, is one of the compounds in kratom. In the leaf it is a minor ingredient. Sold on its own, in tablets, shots and gummies, it is a different drug. The FDA described 7-OH in July 2025 as an opioid that can be more potent than morphine, said it is not lawful in dietary supplements, and recommended it be scheduled as a controlled substance.

Because it is so much more potent, 7-OH withdrawal is a different planning problem from coming off leaf kratom, and men who moved from powder to 7-OH products often describe a habit that got away from them quickly. For planning, the point is simpler: tell admissions exactly which products he has been using, because it changes what the detox needs to look like.

Kratom overdose, and mixing kratom with other drugs

This is the part of the page where waiting is the wrong answer: slow or shallow breathing, a seizure, confusion, or someone who cannot be woken after using kratom or 7-OH is an emergency. Call 911. If he is talking about not wanting to be alive, call 911, or call or text 988.

The FDA warns about seizures, liver toxicity and slowed breathing that can lead to death. NCCIH notes that kratom combined with other drugs has been linked to deaths and severe harm. The combinations that come up on calls here are kratom with benzodiazepines, with gabapentin, with alcohol and with pain medication. Each one adds to the risk, and each one needs saying on the call.

Kratom withdrawal symptoms, and how long kratom withdrawal lasts

Kratom withdrawal is real, and it surprises men who were told they were taking a plant. Because kratom acts on opioid receptors, the FDA lists physical dependence and withdrawal among its opioid-like effects, and men describe it to admissions in the same terms people use for coming off pills. We are not going to publish a list of kratom withdrawal symptoms or a kratom withdrawal timeline, because how long it lasts and how hard it is depends on the product, the amount, how long, and what else is in the picture. What matters for planning is that the days after he stops are when he is most likely to go straight back to the counter, and that is the stretch where being somewhere structured matters most.

Kratom detox, and why coming off at home goes wrong

Families often try kratom detox at home first. A son or a grandson at home, sleeping badly, up and down emotionally, with someone checking on him. Sometimes it works. Sometimes it ends with a trip back to the smoke shop within days, and it is worth knowing the risks before you try it.

Holdfast is not a detox facility. For heavy daily use, 7-OH products, or kratom mixed with benzodiazepines, alcohol or other drugs, admissions will usually recommend a medical detox first. They find one, speak to the intake nurse for you, and collect him when he is medically clear so he comes straight into treatment. Suboxone for kratom withdrawal

Some men arrive here having been started on buprenorphine, sometimes as Suboxone and sometimes as a monthly injection, to get them off kratom. That is a medical decision for the detox physician or prescriber, not something to arrange yourself. If he is already on it, it continues while he is in the program. Kratom and alcohol, and kava and kratom

Kratom rarely travels alone. Kratom and alcohol together is common on these calls, and so is the kava bar, where kratom tea sits on the same menu as kava and both are sold as relaxing and natural. A man who drinks kava and kratom every evening is often not using the word addiction about either.

If alcohol is part of it, that matters for detox, because stopping alcohol can be dangerous in a way that needs a medical setting. Say everything on the call.

Kratom for pain, and what it was doing for him

Kratom for pain is how a lot of these stories start. NIDA says people report using kratom to manage pain, anxiety, depression and opioid withdrawal. The pain is often real. The kratom was a way to manage it without a doctor, and by the time he calls it has usually become its own problem on top of the one it was meant to fix.

So treatment here takes the thing underneath seriously. That starts with taking the pain seriously in the assessment rather than dismissing it. It often means anxiety, depression or trauma he was quietly medicating, treated at the same time by the same clinicians because we are licensed for dual diagnosis. Around ninety days, stepping down through levels rather than ending on a date:

• Residential, at AnchorPoint, our residential program in Prescott. Living on site, full days, the most structure.
• Partial hospitalization, clinical hours most of the day, with housing in the evenings.
• Intensive outpatient, fewer hours, with work starting to fit around them.
• Outpatient and sober living, where the structure comes off slowly rather than all at once.

Kratom is sold in smoke shops, vape shops and online, which makes the months after treatment harder than people expect. Ninety days gives a man time to rebuild sleep and routine, to work out what he was managing with it, and to leave with a plan for walking past the counter. Inside that: psychiatric care on site, non-narcotic support for sleep, CBT, DBT and EMDR, group work and the twelve steps with a real sponsor.

How to quit kratom when the family says it is harmless

Many of these calls come from a mother or a grandparent, and often part of the family does not think it is a real drug.

You do not need him to agree before you start. Admissions can check his coverage, tell you which level of care fits and hold a plan ready while he is still saying no. That is how admissions works.

Do not argue about whether it is natural. Argue about what it is costing. The money. The year. The FDA's own warnings are there if anyone needs them.
